How to Report Fraud and Abuse

You can report fraud and abuse of the Medicaid program and its related benefits using the Quick Links on the AK MMIS Health Enterprise Portal Home page.

The Fraud and Abuse page provides you with examples of behaviors or practices that may be considered fraudulent in nature and that should be referred to the state for further investigation and instructs you on what information should be reported to the state when fraud is suspected. You should provider the name and address information, as well as any other defining information that may be known, of the party suspected of committing fraudulent acts relating to services paid for by the Medicaid Program.

To report fraud and abuse

1. On the Home page, go to the Quick Links pod, and then click Report Fraud and Abuse.

   An instructions page is displayed, with a link to a complaint form.

2. To report fraud and abuse of the program, do one of the following:
   • Call the telephone number provided.
   • Fax the information to the telephone number provided.
   • Mail the information to the address provided.
   • Complete the online Fraud and Abuse form. You must have Adobe Reader on your computer to access the online complaint form.
