How to Request a User Name or Password Reset

Providers or internal users that forget their user ID or password must contact their Organization Administrator. This is the person in your organization who sets up and maintains logon accounts.

Providers

• Please contact your Organization Administrator, the person listed on your enrollment application if you registered at the time of enrollment.

Internal Users

• Please contact your Organization Administrator, the person in your organization that sets up and maintains your logon account.
